As part of the on-going CRF Frozen Foods recall, The Pictsweet Company is notifying consumers that certain Kroger brand frozen vegetable sold in Frys, King Soopers, City Market, Ralphs, Food 4 Less, Smiths, QFC, and Fred Meyer are included in the recall announced Friday, May 6. These products were packaged by The Pictsweet Company and contain Green Beans or Green Peas recently recalled by CRF Frozen Foods due to a health risk.

The Kroger Co. (NYSE: KR) said today it is recalling Kroger Broccoli Raisin Salad because a supplier indicated sunflower kernels included in the package may be contaminated with Listeria monocytogenes, an organism which could result in severe illness to those individuals who may consume this product. No customer illnesses have been reported to date.

Let’s Talk Health, Inc., is recalling its 15 packet jars of “Energizer Drink Mix,” Agua Vitae Liquid Multi-Vitamin, Agua Vitae Kids Liquid Multi-Vitamin and Tox-Elim because they contain an undeclared milk and soy allergen from Whey Protein. People who have allergies to soy and milk run the risk of serious or life-threatening allergic reaction if they consume these products.

Bickel’s Snack Foods is voluntarily recalling 2,010 bags of Golden Gourmet Veggie Chips. The Golden Gourmet Veggie Chips are coated in Ranch Seasoning made with milk. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume these products.

Ajinomoto Windsor, Inc. is voluntarily recalling various Not-Ready-To Eat frozen food items due to the potential for these products to be contaminated with Listeria monocytogenes. This voluntary action is being undertaken in cooperation with the United States Department of Agriculture and the US Food and Drug Administration because the recalled products contain vegetables that are part of the recent CRF Frozen Foods recall.

Out of an abundance of caution, Harris Teeter is voluntarily recalling Harris Teeter Frozen Organic Corn and Harris Teeter Frozen Mixed Vegetables due to the potential for these products to be contaminated with Listeria monocytogenes.

PharMEDium Sterile Preparations Compounded With a Single Recalled Lot of Fresenius Kabi Sensorcaine-MPF (bupivacaine HCl): Recall – Presence of Glass Particulate Matter

AUDIENCE: Pharmacy, Risk Manager
 
ISSUE: PharMEDium Services, LLC is voluntarily recalling the codes/lots of sterile preparations compounded with a single recalled lot of Fresenius Kabi Sensorcaine-MPF (bupivacaine HCl) to the hospital level. Fresenius Kabi recalled the specific lot at issue due to identification of particulate matter, characterized as glass,  during reserve sample inspection. Fresenius Kabi indicated in its recall notice that the presence of glass particulate matter in solution, if undetected and administered via the epidural route, could block drug administration, delaying therapy. If the particulate is able to pass through the catheter and enter the body, this could result in local inflammation, mechanical disruption of tissue or cause an immune response to the particulate.

BACKGROUND: The drug products affected by this recall are indicated for the production of local or regional anesthesia or analgesia for surgery procedures, diagnostic and therapeutic procedures and for obstetrical procedures. The product was distributed to hospital customers and acute surgery centers in the United States.

RECOMMENDATION: PharMEDium Services has contracted with Inmar Pharmaceutical Services to coordinate this recall. Inmar will send notification via electronic mail to all affected customers, request an accounting of remaining units, quarantine the affected products and ensure the destruction of products.

Hospital pharmacies or acute surgery centers that have one or more of the products referenced in this notice should immediately remove from all points of use and quarantine in accordance with applicable hospital policy. Hospital pharmacies or acute surgery centers that may have shared these products with other hospitals should contact those hospitals that received the products.

Healthcare professionals and patients are encouraged to report adverse events or side effects related to the use of these products to the FDA’s MedWatch Safety Information and Adverse Event Reporting Program:
